Wellness Membership Fees

(there is a ONE TIME $75 intake fee, in addition to membership on the initial visit)

INDIVIDUAL PRICING:

Newborn to 17 years

  • $50/month.

  • ^Discounted to $25/month when accompanied by an adult membership.

Age 18 years- 44 years

  • $80/month. ^

Age 45- 64 years (or until the patient is on Medicare)

  • $100/month ^

    FAMILY PACKAGE (2 adults and up to 3 children):

    $200/month ^

    +$15 each additional child ^

    *We ARE NOT health insurance and recommend at a minimum a high deductible insurance plan or health-sharing account for unforeseen medical issues and emergencies. *

    ^ Fees will be billed monthly as a recurring payment on the first Wednesday of each month. All fees for service are required to be paid at the time of service. A 30-day written cancellation notice is required to cancel a membership.

    *Initial intake appointments can take up to 60 minutes to allow time to thoroughly get to know one another and explore details of current and past medical history. This 60-minute visit does not include the time to register in our communication service, review records, register you in our EHR, and billing subscription set up. Those services and time are covered by the initial intake fee.

    **Upon enrollment members will be added/given access to our communication service with the ability to have 24/7/365 direct access to the provider. From this service we are able to chat, text, talk on the phone or have a video call whenever it’s needed. The provider will respond to urgent text messages/calls within 1-2 hours. If it is a non-urgent matter we will respond within 12-24 hours. This method of open communication is what makes Serendipity different. You have your provider available to you at the touch of a button. We do ask that you try to avoid non-urgent calls and text messages between the hours of 10 pm-8 am EST. In the event of an emergency, we will ALWAYS advise calling 9-1-1 or heading to the nearest emergency room.
